Urgent readings come through

Quiet modes reduce noise, and a reading that crosses a safety threshold goes to the top of your Action Center every time.
An engine that watches everything will, by default, say too much. The fix is to be strict about what counts as urgent, and unfiltered about it.
Quiet by design
Most notes can wait. The engine deduplicates, caps the number of notes per category, and respects modes you set, such as rest days after illness. The engine can hold a note about a small shift in sleep until tomorrow.
Urgent by rule
Some readings need attention now, such as blood pressure at or above 180/120. The engine raises it as a critical alert at the top of your Action Center, past every quiet mode, cap and deduplication rule. The engine enforces this in code, so the rule holds whatever the settings say, and a care team can only lower the threshold, never raise it.
What the note says
A critical alert says what the reading is, what threshold it crossed, and what to do next: rest, measure again, and seek care if it repeats or you have symptoms. Causes are a conversation for a clinician, and the alert says so.
